Most neonates successfully meet the challenge presented by labor, the delivery process, and subsequent adaptation to the extrauterine environment. Birth is the riskiest stage of life because failure of postnatal transition can result in immediate physiologic instability, disability, or end-organ dysfunction and death. Approximately 10% of neonates require some resuscitative assistance during transition in the delivery room. For example, 90% of very low-birth-weight newborns (< 1,500 g) require immediate respiratory support; although most of these neonates require only continuous distending pressure or oxygen, approximately 6% receive advanced resuscitation, including chest compressions.
