The respiratory tract is continuously exposed to pathogens through inhalation. To protect against infection, the lungs have a variety of defense mechanisms, including a mucociliary barrier, the innate immune system, and the adaptive immune system. This chapter focuses on pulmonary complications arising from congenital and acquired defects in innate and adaptive immunity. This chapter does not address the diagnostic workup of suspected immunologic conditions, which should be undertaken in collaboration with an experienced immunologist.
