In 2014, the Current Procedural Terminology (CPT) code set added new codes to report complex chronic care coordination services, also known as C4 services. CPT has revised this code set for 2015 and created a new category titled “Care Management Services,” which is broken down into two subsections titled “Chronic Care Management Services” and “Complex Chronic Care Management Services.” These are complex codes with many guidelines and restrictions, so it will be vital to understand all aspects of the codes and their respective uses.
