A new toolkit is available to help improve the safety of infants born prematurely or with complex congenital conditions as they transition from the neonatal intensive care unit (NICU) to their home.

Transitioning Newborns from NICU to Home features information on how to create an online health coach program. The health coach serves as a teacher and facilitator who encourages communication with the parents/caregivers to identify needs and concerns and facilitates follow-up care for the infant by primary care providers. The online coaching program can customize information based on a family’s needs and concerns. Approximately 30 fact sheets for clinicians and families are available on topics such as medications, breastfeeding and insurance coverage tips.
