New clinical resources and a redesigned website have been released to help practices with patients transitioning from pediatric to adult health care.
The resources are from the Center for Health Care Transition Improvement — Got Transition. The center recently released Six Core Elements of Health Care Transition (Ver. 2.0) to define the basic components of transition support. Guidance is based on the AAP clinical report, Supporting the Health Care Transition from Adolescence to Adulthood in the Medical Home.
Transition tool packages are available in English and Spanish for practices serving youth who will transition out of pediatric care into adult care; practices serving youth who will remain with the same provider but need to transition to adult-focused care; and practices accepting new young adults into adult care.
