Manufacturer: Tots in Mind Inc., of Salem, N.H.

Hazard: Clips that attach the tent to the top of the play yard can break or be removed by a child. The child then can lift the tent and become entrapped between the play yard and the tent, posing a strangulation hazard.

Description: The Cozy Indoor Outdoor Portable Playard Tents Plus Cabana Kits have 12 plastic clips that secure the tent to the top rail of the play yard to contain a child. The tent has a zippered side for putting in and taking out the child.

Incidents: A 2-year-old boy died when his neck became trapped between the play yard frame and metal base rod of the tent. The tent had been partially tied to the play yard with rope because the child was able to pop off the clips.

Sold at: Walmart, Amazon.com and various baby and children’s stores from January 2005 through February 2010 for about $60.
