Product recall includes pacifier clips, crib mattresses :

The U.S. Consumer Product Safety Commission (CPSC) has announced the recall of the following products. Consumers should stop using recalled products unless otherwise instructed. Consumers can submit reports of harm to CPSC’s searchable online product safety database at www.SaferProducts.gov. A searchable food and medical product recall database is available at www.fda.gov/Safety/Recalls/default.htm.

 

De-icing products

Units: About 4.1 million

Hazard: The trigger spray assembly can be removed from the container. The product can be poisonous to children who swallow it.

Description: This recall involves two products: Prestone Windshield De-Icer and Prestone Ice & Frost Shield. The de-icer (SKU AS-246) is used to melt ice on vehicle windshields and windows. The ice and frost shield (SKU AS-247) is used to reduce morning frost, light ice and snow on vehicle windshields, windows and wiper blades. Both were sold in a clear yellow 32-ounce trigger spray plastic bottle.

Sold at: AutoZone, Kroger, Meijer, O’Reilly’s, Pep Boys and Wal-Mart, online at www.Amazon.com, and other retailers from September 2011 through August 2015 for $3 to $11.

Remedy: If the trigger assembly can be removed by twisting the neck counterclockwise, contact Prestone to receive a replacement trigger assembly that cannot be removed when it is engaged.

Bicycle quick release levers

Units: About 1.3 million

Hazard: An open quick release lever on the bicycle’s front wheel hub can come into contact with the front disc brake rotor causing the front wheel to come to a sudden stop or separate from the bicycle.

Description: This recall involves bicycles from 13 different companies. The bikes are equipped with front disc brakes and a black or silver quick-release lever on the front wheel hub. Bicycles that do not have disc brakes are not included in this recall. A video showing how to determine if a bike is included in this recall is available at www.quickreleaserecall.com.

Sold at: Bicycle stores nationwide from about 1998 through 2015 for $200 to $10,000.

Remedy: Contact the recalling company for free installation of a new quick release on the front wheel.

Water bottles

Units: About 178,000

Hazard: The plastic straw in the flip-top portion of the cap can break. Children using the bottle can get plastic fragments in their mouths.

Description: The recall involves Zak Designs 26-ounce plastic water bottles, which have a flip-top spout on a twist-off cap. Straws come in clear, blue, gray, green, light purple or red. The bottles have popular characters on the front, including Captain America, a Batman design, Minions, My Little Pony, Spiderman, Star Wars aircraft, Superman logo, Teenage Mutant Ninja Turtles, Thor and Wonder Woman. Colors include clear, gray, navy blue, pink, teal blue and gray, with caps in red, blue, pink and yellow. Number 14158 and “Zak Designs” are embossed on the bottom of the bottles. Bottles with a black straw and black twist-off cap are not recalled.

Sold at: Target stores in June and July for about $10.

Plastic trapeze rings

Units: About 121,000 pairs

Hazard: The rings can unexpectedly crack or break during use, posing a fall hazard to children.

Description: Recalled are yellow plastic trapeze rings, which are triangular in shape with rounded sides and a loop at the top. Rings were sold as a separate component or an attachment on the following Rainbow-branded residential wooden playsets: All-American, Backyard Circus, Carnival, Fiesta, King Kong, Monster, Sunray, Sunshine and Rainbow.

Sold at: Rainbow dealers nationwide from January 2007 through December 2011 and at retailers, including Sam’s Club, Toys R Us and Wal-Mart from January through December 2009.

Remedy: Contact Rainbow for ring removal instructions, then remove the rings from the playset and receive a $10 gift card.

Pacifier clips

Units: About 45,000

Hazard: The “D” ring on the pacifier clip can break, allowing beads to detach. Children can choke on the beads.

Description: Five styles of Chewbeads Baby pacifier clip holders are recalled. The “Where’s the Pacifier?” style includes a butterfly, dinosaur, heart, sheriff badge or a white baseball with red stitching and a Major League Baseball team logo. “Chewbeads baby” is imprinted in raised letters on the rubber shape attachment. The pacifier clips have eight multi-colored beads threaded through a narrow satin ribbon that attaches to a pacifier on one end and a plastic D-ring on the other. One of these number codes is on back of the garment clip: 3/31/14, 08/08/14, 12/01/14, 14/30/09, 15/02/09, 25/04/14.

Sold at: Buy Baby and small boutique retail stores nationwide from September 2014 through June 2015 for $15 to $16.50.

Crib mattresses

Units: About 38,400 (VYSSA SPELEVINK crib mattresses were previously recalled in January and again in May for entrapment hazards.)

Hazard: The crib mattresses pose a fire hazard.

Description: This recall involves IKEA VYSSA SPELEVINK crib mattresses. Mattresses that were given in exchange to consumers for their VYSSA SPELEVINK mattresses in the two previous recalls are included in this recall program.

Sold at: IKEA stores nationwide and online at www.ikea-usa.com from October 2000 through May 2014 for about $100

Remedy: Return them to any IKEA store for a refund.

Fishing games

Units: About 14,000

Hazard: The plastic worm at the end of the fishing pole can separate, producing small parts that children can choke on. The small magnet inside the worm also can come loose. Swallowing multiple magnets can result in serious internal injury.

Description: This recall involves two models of the Juratoys fishing game, Sardines and Starfish. Each set comes with two wooden fishing rods and several wooden fish with a magnetic button in the middle. “Janod” is printed on the lid of each tin package.

Sold at: Online at www.citruslane.com, www.burrogoods.com, www.terratoys.com, www.patinastores.com and other retail stores from April through August for $15 to $20.

Remedy: Contact Juratoys for a prepaid shipping envelope to return the game. Juratoys will send a refund check.

Cribs

Units: About 6,000 (Bexco recalled an additional 11,600 Bexco cribs in July)

Hazard: A metal bracket that connects the mattress support to the crib can break, creating an uneven sleeping surface or a gap. A baby can become entrapped in the crib, fall or suffer cuts from the broken metal bracket.

Description: The recall includes DaVinci brand full-size cribs, including the Reagan crib (model #M2801), the Emily crib (model #M4791), the Jamie crib (model #M7301) and the Jenny Lind crib (model #M7391) manufactured from May through December 2012. The model number, serial number and manufacture date are printed on a label affixed to the bottom right side panel of the crib. Cribs newly included in the recall have serial numbers that begin with “N00,” followed by one of the following numbers: M2801 Reagan: N00 + 5254, 5350 or 5035; M4791 Emily: N00 + 5249, 5380, 5534 or 5193; M7301 Jamie: N00 + 5108 or 5381; and M7391 Jenny Lind: N00 + 5214, 5100, 5227, 5313, 5382, 5094, 5524 or 5644. Sports hoodie

Units: About 1,200

Hazard: The sweatshirts have a drawstring around the neck area, which can become entangled or caught on playground slides, hand rails, school bus doors or other moving objects.

Description: This recall involves James Trading Group’s Croker Kids Ireland Sports Hoodie. The kids’ hoodie has green fabric on the upper chest and arms with a white diagonal stripe across the torso and a blue panel on the bottom half of the hoodie. “Ireland” is printed in white letters across the chest, and three Shamrock logos are embroidered onto a patch on the bottom left side of the hoodie. The hoodie was sold in kids’ sizes “2YR” through “12YR.”

Remedy: Remove the drawstring to eliminate the hazard or return it to the store where purchased for a refund.

Recycled bikes

Units: About 400

Hazard: Fingers placed at the center where the handlebar connects to the bike frame can get pinched.

Description: The Wishbone Recycled (RE) bikes are made from recycled black plastic materials with 12-inch white rubber tires. The two recalled bikes include one 3-in-1 model, which is adjustable as a 3-wheeler or 2-wheeler with a high seat or low seat; and one 2-wheeler model, which is adjustable with a high or low seat. The date codes, which appear in a round dial on the front frame under the seat, are either December 2013 or May 2014.

Sold at: Independent toy and bike stores nationwide and online at www.amazon.com from July 2014 through June 2015 for about $200 for the 2-wheeler and $230 for the 3-in-1.

Remedy: Contact Wishbone or the store where the bike was purchased for a free neoprene cover for the handlebar.

Sheep mobiles

Units: About 400

Hazard: The yarn from the sheep figures can unravel, and young children can get tangled or strangle.

Description: This recall involves The Land of Nod Follow the Herd mobile, which is made of white wool felt and has five sheep figures made of white wool yarn. SKU number 198234 is on a label attached to the body of the mobile.

Sold at: The Land of Nod stores nationwide and online at www.landofnod.com from September 2013 through May 2015 for about $50.
