The purpose of the visit was to receive counseling on disease and injury prevention during travel. The preventive medicine counseling and/or risk factor reduction codes 99401–99404 describe physician and other qualified health care professional services to patients without a specific illness. Reporting of preventive medicine counseling is based on the amount of time spent providing face-to-face counseling with the patient and/or caregivers. In this scenario, the physician spent 25 minutes counseling the patient and family. This supports reporting of code 99402, which has a time of approximately 30 minutes. Code selection is based on the Current Procedural Terminology (CPT®) instruction that the code with the higher time is reported when the midpoint between the times assigned to 2 codes in a series of time-based codes is passed.
