Coding Clinic for ICD-10-CM and ICD-10-PCS has advised that because code K31.89 does not specifically identify the disease process of cyclical vomiting syndrome, it is appropriate to assign an additional code from category R11 to capture the vomiting.

This is subsequent care and is reported with the appropriate injury code ending with seventh character D (eg, S61.412D, laceration of left hand without foreign body, subsequent encounter). Although this was your initial encounter with the injury, the care took place during the healing phase, rather than active treatment.
