The 2020 updates to Current Procedural Terminology (CPT®) bring good news to pediatric surgeons and urologists who perform orchiopexy. The code descriptor for code 54640 (Table 1) has been revised in 2 important ways.

Inclusion of the additional scrotal approach to performing the procedure is important. However, this is fairly straightforward. It is important that pediatricians not miss the deletion of “with or without hernia repair” and what this is intended to convey— hernia repair is separately reportable when performed in conjunction with orchiopexy.
This change corrects a coding recommendation published in the June 2008 issue of the American Medical Association CPT Assistant regarding the appropriate reporting of a procedure that includes both an orchiopexy and inguinal hernia repair when performed in the same incision.
