Screening for alcohol and drug use (and tobacco use) is recommended by Bright Futures guidelines for all adolescent patients. Services recommended in the Bright Futures guidelines are typically covered without an out-of-pocket cost to the patient or caregiver. However, the context of how and when the service is delivered affects code selection and payment. Counseling, anticipatory guidance, and risk factor reduction intervention are components of a comprehensive preventive medicine visit (new patient, 99381–99385; established patient, 99391–99395) and cannot be billed separately on the same date of service. Do not report codes 99401–99404 and 99411 or 99412 for preventive medicine counseling and/or risk factor reduction on the date of a comprehensive preventive medicine visit.
