CPT®: Multiple Modifiers and Telemedicine Services

Because some modifiers have an effect on how a service is paid and some are reported to provide additional information that does not affect payment, it is preferable to place the modifier that affects payment first. In this scenario, modifier 25 should be placed before modifier 95, as payment for the service may depend on the use of modifier 25 to indicate the E/M service was significant and separately identifiable from another service reported on the same date. Modifier 95 may also affect the amount paid for a service but does not override claims edits. Also, note that some payers require modifier CR (catastrophe/disaster related) on services provided to patients in their homes via telemedicine during the public health emergency. Modifier CR is informational and should be placed after modifiers that affect payment.
