The appropriate codes must be used to identify diagnoses, symptoms, conditions, problems, complaints, or other reasons for the encounter and should be supported by documentation in the medical record. Because you (or another physician in the same practice) had previously provided professional services to this newborn, the neonate would be an established patient.
