Over the past month you have reviewed the consultation report from the pediatric endocrinologist and revised the care plan to include the new diet, glucose testing, and insulin requirements. Time was required to order and review laboratory reports including the home glucose monitoring log and communicate with the parent via telephone. In addition to the documentation in the medical record you have recorded your time on the care plan oversight worksheet that is maintained as part of the medical record and for use in billing.

In this example, the total time spent to provide care plan oversight services during the calendar month was 31 minutes. Therefore, Current Procedural Terminology (CPT®) code 99340 (home care plan oversight services, 30 minutes or more) should be reported.
