The medical team conference codes will be used to report participation by physicians or nonphysician health care professionals (eg, speech-language pathologists, audiologists, physician assistants, pharmacists, physical therapists, occupational therapists, registered dietitians, geneticists, genetic counselors, psychologists, social workers) in conferences to coordinate or manage care and services for established patients with chronic or multiple health conditions (eg, cerebral palsy) or with congenital anomalies (eg, craniofacial abnormalities).

The 2008 Current Procedural Terminology (CPT®) guidelines stipulate that a minimum of 3 qualified health care professionals from different specialties or disciplines (ie, only one participant of the same specialty) who are actively involved in the patient’s care must participate in the conference. Each participating health care professional must have performed a face-to-face evaluation or treated the patient within the previous 60 days of the conference.
