Our practice will be open and seeing scheduled patients on July 3, 2009. Most federal offices are closed on that date in observance of Independence Day (July 4 falls on a Saturday). Is it appropriate to report Current Procedural Terminology (CPT®) code 99051 (services provided in the office during regularly scheduled evening, weekend, or holiday office hours, in addition to basic service)?

CPT code 99051 has liberal guidelines. CPT does not define the term holiday, thus leaving the definition open to payer interpretation. It would appear that using federal holidays as a guideline would be appropriate. However, you should contact your payers for specific clarification on their interpretation and definition of holidays.

Keep in mind that code 99051 and the other after-hours codes (99050–99058) are reported in addition to the basic service. The basic service could be an evaluation and management (E/M) service code (eg, 99201–99205, 99281–99285...
