Which Current Procedural Terminology (CPT®) code should we report for a lidocaine injection administered prior to removing a foreign body from the foot?

CPT codes 28190 (removal of foreign body, foot; subcutaneous), 28192 (deep), and 28193 (complicated) include local infiltration, metacarpal/metatarsal/digital blocks, or topical anesthesia as part of the global surgical and procedure package. CPT and the Centers for Medicare & Medicaid Services have different definitions of a “surgical [procedure] package.” It is important that practices understand the requirements for reporting surgical procedures and understand which guidelines are followed by state Medicaid programs and commercial payers.
