The Healthcare Common Procedure Coding System (HCPCS) includes Level I Current Procedural Terminology (CPT®) codes, Level II Centers for Medicare & Medicaid Services national codes, and Level III local codes assigned and used by Medicare carriers. Level II codes (commonly referred to as HCPCS ["hick-picks"] codes) are used to report services and supplies that are not identified within CPT.

HCPCS codes J0100–J9999 are used to report medications that are administered in an outpatient setting (ie, physician's office, urgent care center, and hospital outpatient department). The codes are specific to the drug, route of administration (eg, injection, intravenous, oral), and dosage. The majority of J codes include the brand name and many codes include specific reporting instructions.

Refer to the Table of Drugs in your 2011 HCPCS book (example in Table 1). Note that some drugs are listed with a generic name and others with a brand name.
