For physicians reporting performance measures, the Current Procedural Terminology (CPT®) Alphabetical Clinical Topics Listing (formerly know as Appendix H) has been revised for asthma by the measure developer.

CPT codes 1005F (asthma assessment) and 4015F (asthma pharmacologic therapy) have been deleted. The following new and revised codes are effective for services provided on and after January 1, 2012.

New asthma measures apply to all patients with a diagnosis of asthma who are 5 to 50 years of age.

The following codes have not been deleted. However, note that these codes are not associated with any measure.
