Correct coding is essential for appropriate payment as well as tracking morbidity and mortality and statistics related to health care costs and disease management. Coders and physicians must understand International Classifications of Diseases, Ninth Revision, Clinical Modification (ICD-9-CM) coding conventions, as they are essential for the correct selection of many codes. Likewise, it will be just as essential for physicians and coders to follow the conventions applicable to International Classification of Diseases, 10th Revision, Clinical Modification (ICD-10-CM) when it is implemented. ICD-9-CM and ICD-10-CM include basic information for physicians and hospitals. The official conventions are outlined in sections that include descriptions of symbols, abbreviations, and other instructional notes, followed by coding guidelines specific to physician and hospital coding. Guidelines for physician coding are found in sections I and IV. Physicians and coders must be familiar with these coding conventions and guidelines.
