For the majority of your patients, non–face-to-face care plan oversight activities may be infrequent and of low intensity. However, patients with special health care needs often require complex and frequent non–face-to-face care plan management and coordination. This work can be captured for billing by tracking the time spent and reporting a care plan oversight code that corresponds to the type of management and time spent each calendar month. It is a bit of work to develop a process for tracking the time spent and capturing codes for billing, but this allows you to get paid for the work you are already doing. Electronic health record capabilities may ease this burden if a template for capturing care plan oversight with fields for start time, description of work, stop time, and total time can be used to capture each physician's activity and provide a monthly report by patient and total time.
