International Classification of Diseases, 10th Revision, Clinical Modification (ICD-10-CM) code revisions and guidelines went into effect October 1, 2018. New and revised codes were discussed in the August 2018 AAP Pediatric Coding Newsletter™ ( In this article, guideline changes that may affect reporting of pediatric diagnoses are reviewed.

The guideline for reporting certain codes based on documentation by clinicians other than the treating physician or other qualified health care professional (QHP) is amended to state that, except for certain information typically documented by other clinicians involved in the care of the patient, code assignment is based on the documentation of the treating physician or QHP. The guideline also now includes social determinants of health as reportable when documented by a clinician other than the treating physician or QHP. Changes are noted in bold text.
