Participants identified root causes, organized into the categories of supplies, policy, people, environment, equipment, and process. Responses from multidisciplinary team members were consolidated. The primary root cause, determined by consensus, was the inability to provide high-quality CPAP.

Key driver diagram. A key driver diagram was developed to link the global aim, reduction in preventable deaths of preterm infants admitted to Ethiopian NICUs, with the telementoring intervention focused on standardizing and improving care of preterm infants with RDS. RDS is a primary driver of preventable death in preterm infants, with secondary drivers identified as optimizing use of CPAP and standardizing assessments for RDS. Change ideas linked to the secondary drivers were proposed for implementation in this collaborative.

Initial diagnostics: (A) initial electrocardiogram after stage 2 hypospadias repair showing significant ST segment elevation in anterolateral and inferior leads. (B) Initial right coronary angiography: the right coronary artery has severe narrowing throughout its distribution with a thread-like proximal course, which dilates to a trifurcation of vessels supplying the right anterior free wall. (C) Initial left coronary angiography: left main coronary artery appears to be of normal shape with no wall irregularities. The left circumflex has a posterior course and supplies the posterior descending, making this a left-dominant system. The left anterior descending does have a takeoff anteriorly; however, there is no further advancement of contrast through the expected distribution of the left anterior descending. Instead, there are at least 3 small collateral vessels with varying degrees of perfusion and filling presented throughout the injection. There is evidence of attempted collateralization into the territory of the left anterior descending. The distal left circumflex vessels also appear to be significantly pruned or narrowed.

Repeat coronary angiography: (A) hypoplastic right coronary artery taking the usual course down the anterior right atrioventricular groove. There is a sinoatrial nodal branch appreciated and an interior inferior branch that is not as well filled in this injection; however, we can see again multiple diffuse small skipped lesions with revascularization from the periphery. (B) Left main gives rise to the circumflex and left anterior descending artery (LAD). Almost immediately after the takeoff of the LAD, there is total occlusion with multiple small branches attempting revascularization of the territory of the LAD. There are multiple skipped lesions in the small vessels off the branches of the LAD suggesting multivessel, multiple areas of small vessel disease. There was a diagonal off the circumflex as well as a proximal diagonal that also demonstrates this long segment narrowing. The circumflex is of good size otherwise, although multiple areas of small vessel involvement in the periphery remained.
