A twin 1 female infant with a birthweight of 1,354 g is delivered at 30 4/7 weeks by normal vaginal delivery to a 29-year-old primigravida mother. The Apgar scores are 6 and 8 at 1 and 5 minutes, respectively. The mother has a history of vaginal leaking before 12 hours of delivery and has received 1 dose of antennal corticosteroid. This is an in vitro fertilization conception with no history of consanguinity. Antenatal history is uneventful except for the maternal history of vaginal leaking since the 19th week of gestation. The infant receives continuous positive airway pressure (CPAP) in the delivery room and is transferred to the NICU where CPAP is continued after intubation-surfactant-extubation.
