On page e1680, first paragraph, fourth sentence should read: This intake should be increased to about 400 IU/day (maximum 1000 IU/day) when the infant is tolerating full enteral nutrition.
On page e1682, summary item 7 should read: When infants are tolerating full feeds, vitamin D can usually be started at about 400 IU/day up to a maximum of 1000 IU/day.
